Student Travel Forms

If staying overnight on an LBCC sanctioned activity/event/class, the following forms would be used:

  1. Faculty/staff fill out the "Notice to Safety & Loss Prevention of overnight stay"
  2. Have students (and volunteers) complete the "Risk Acknowledgment Waiver" for the activity
  3. Have students, staff, and volunteers complete the "Commitment to Comply" form
  4. If there are students or staff driving others to the activity, they must submit the "Driver Status Form" at least 10 days in advance for MVR and approval processing.
  5. If driving a van for the activity, the driver needs to complete the van driver training as well
  6. There is a Travel Preparation Checklist that has a tab of preparations to have in place for international travel (staff tab and student tab) and a tab for domestic travel.

If going out of country, complete the above forms plus:

  1. The "Emergency Contact and Information Form"
  2. Staff/faculty needs to submit a list to the Director of Safety and Loss Prevention of those going  (a spreadsheet will be provided with the necessary information needed)  for the purchase of international medical/accident insurance (This is necessary to ensure medical care can be obtained in a foreign country. It’s very inexpensive and provides excellent coverage abroad). 

For regular class or club activities in the area but not overnight the following forms must be completed:

  1. Have students (and volunteers) complete the "Risk Acknowledgement Waiver" for the activity
  2. Have students, staff, and volunteers complete the "Commitment to Comply" form
  3. If there are students or staff driving others to the activity, they must submit the "Driver Status Form" at least 10 days in  advance for MVR and approval processing. 
  4. If driving a van for the activity, the driver needs to complete the van driver training as well.
